Multiple attachments per joint

See VWR-489 and SL Attachments Mod
Viewers that don't implement this won't be able to see the other attachments, but perhaps if enough 3rd party viewers use it with no ill effects, and establish it as a standard, we can persuade LL to add support for it in the official viewer, too.
